I'd say the final wave of Iron Man figures that were only made as prototypes, but I know there are a few sets out there, so some would consider those grails I'd guess.

I Just Recently Picked one up and i have never seen it anywere i have only seen ones with Red Cables and One with Green Cables Attachments:
